Rosuvastatin dosing by transporter genotype

Both genes encode transporters that set rosuvastatin's blood exposure — the same OATP1B1 node in the statin-entry overlay, plus BCRP. Decreased/poor function raises drug levels and myopathy risk, so guidance caps the dose. The key SLCO1B1 variant is c.521T>C (*5/*15). Note: a 40 mg dose sits above the recommended starting dose for several decreased/poor-function combinations — worth discussing pharmacogenomic testing with your clinician if it hasn't been done.
